Care Connect offer Private Case Management Services.
Private Case Management and Care Coordination provides individuals with a range of privately contracted user funded care services that are available within the community. These services are available for either short or long term, depending on your individual needs. Including but not limited to, assistance to remain living at home with a level of support that you require; advocacy and support on your behalf; the development of care plans that will identify and support your requirements; assistance with health promotion and education and referral to alternative support services.

Statewide ABI Case Management Service
This service works with people aged from 18 to 64 who have a moderate to severe acquired brain injury (ABI). Including case management for people 5 - 64 on the ABI Slow to Recover program. The sudden onset of an ABI means often clients and their families are not used to dealing with medical treatment, rehabilitation and disability services. Case Managers in this service assist people in accessing services, maximising their quality of life so they can participate in the community as fully as possible.
Statewide ABI Paediatric Coordinators
The state-wide ABI Paediatric Coordinators provide information, mentoring and support to anyone working with a child or young person with a (non compensable) acquired brain injury. They are available to provide training, mentoring support, to assist workers identify suitable services and to provide secondary consultation on individual cases.
Maintaining Access Program (MAP)
MAP provides low-intensity case management to adults (18-64) with an ABI living in metropolitan Melbourne. The role of the MAP Case Manager is to provide ongoing support after a person has received intensive case management. This support is available on a long-term basis. The MAP Case Manager can make sure that the services and supports already in place run smoothly, organise new services if required, assist clients to maintain their independence, and apply for funding to meet specific goals identified by clients.
Melbourne Case Management Service
This program provides case management services to people residing across Melbourne who have compensation (e.g. TAC or Workcover) for their acquired brain injury. It organises assessments and services, develops case plans, and in partnership, assists the client and their families to achieve their goals.

Moira Disability & Youth Services
MOIRA provides a range of support services for people with disabilities and homeless youth in and throughout the Southern Metropolitan Region of Melbourne.
MOIRA services include 3 Case Management programs.
STAPs. Short Term Assistance Programs
Short Term Assistance Packages provide case management and support services to families who are caring for a child or adult (five to sixty five years) with moderate to severe disabilities and or high support needs.
A short term assistance package is designed to provide a timely response to families who require access to appropriate services for their child or adult member with a disability.
A 'Family Services Coordinator will work with the family to ensure they are linked into a range of existing services that meet expressed needs.
Early Choices Program
Early Choices is a family centred program which provides case management and support to families caring for a child (0-6 years) with a severe disability and or very high support needs.
The Early Choices program provides a Family Services Coordinator who works in partnership with families to identify their needs in relation to their child and to find ways of meeting those needs.
A family may be linked with existing services or, where necessary, funds may be available to buy services to meet particular needs. Services provided through Early Choices will be flexible and organised in ways which best suit these needs.
Making a Difference Program
Making a Difference is a family centred program which provides case management and support to families caring for child or adult members (5-65 years) with moderate to severe disabilities and or high support needs.
The impact of disability varies and many families may find they cannot readily obtain the services they require to meet their needs. More flexible services with a wider variety of choices may be required.
The Making a Difference Program provides a Family Services Coordinator who works in partnership with families to identify needs and co-ordinate services and resources to ensure that quality of life is enhanced. Families may be linked into a number of existing services, or funds may be available to purchase service, if necessary.
Eligibility criteria for Case Management Support Services:
- You have a family member living at home aged between 0 and 65 years.
- Your family member has a moderate to severe disability and/or complex support needs.
- Disability may include physical, intellectual, sensory neurological impairment or Acquired Brain injury.

Slow to Recover Program

The ABI STR Program is a Victorian State wide service that provides specialist case management, rehabilitation and support for people aged 5 -64 who have suffered a catastrophic brain injury and require high level or nursing home level of care.
To be eligible for this program the person must have a primary diagnosis of a catastrophic brain injury in relation to an acute health episode within the proceeding 2 years and not be eligible for compensation.

TAC
(Transport Accident Commission)
Case Management is defined by the TAC as a collaborative process to facilitate the client’s achievement of a specified goal, or series of goals over a period of time. The case management process consists of assessment, planning, facilitation, liaison, identifying and recommending necessary options to address individual needs, maximise independence and achieve quality cost-effective outcomes.
Eligibility: Only TAC claimants who have sustained a severe injury as a result of a transport accident e.g. severe brain injury or spinal cord injury
Referral process: TAC Support Coordinator (SC) can complete referral. Clients or providers should contact SC initially to discuss whether CM is reasonable.

Yooralla
People Helping People Achieve
ISP Facilitation
Individualised Support Packages (ISP) are allocated via DHS to people with a disability who have an ongoing disability need. The ISP provides funding to purchase supports to assist with this disability need so that the individual can access their local community and participate actively in their life choices. Yooralla ISP Facilitation enables people with a disability, who want to enjoy community life and expand their opportunities, to explore their goals and needs and how they can be met. Planners work with individuals, learning about their ambitions and gaining a full understanding of their issues and what might need to be done to resolve them. From this shared understanding a plan is developed. The plan outlines the desired lifestyle and lays out the steps that will need to be taken to achieve it. A funding proposal is then developed to purchase the required services (within the notional allocation approved by DHS). This service is funded by DHS as part of the ISP process and an individual can choose Yooralla to facilitate their ISP.
Specialised Case Management & Consultancy Service
Yooralla's Specialist Case Management is a management service that works in partnership with the client, and where appropriate, the family; to provide holistic intervention to explore life-time goals and ensure the right types of support are in place. The Case Manager wants to understand what is important to the client. They will discuss what is most appropriate to the client’s abilities, needs and goals, and what supports will be required to resume and maintain an active role in the community (immediately and in the long-term). The Case Manager will assist the client to develop an individual plan that identifies how various services and supports work together towards achieving the client's goals. The Case Manager works with the individual to put their future plan in action and review progress. Our service approach is based on person-centred practice.
